Test forms (local authority users)

The Test forms website allows schools to apply for access arrangements for national curriculum tests online.

Local authority assessment advisers are responsible for:

  • approving and managing access arrangements applications and notifications from schools via the website
  • assisting school users with technical problems
  • ensuring the site is used securely and responsibly in their area.

Local authorities must review and approve each application for access arrangements that is submitted by schools in their area through the Test forms website. They must base decisions to approve or reject applications on the criteria published in the Assessment and reporting arrangements.
